She is preceded in death by her parents, William and Dianne Smith; brother, Robert Smith.
She is survived by daughter, Kourtney Perez of Nashville, TN; sons, Zachary Johnson, of Hot Springs, AR and Gavin Johnson of Brownsburg, IN; brother, Brian Smith of Roscoe, IL; and ex-husband, Kevin Johnson also of Brownsburg, IN.
A memorial service will be conducted on Saturday, April 27th at 11:00am at the Point of Mercy Church, 200 Gleaves Street, Madison, TN 37115
